Help with figuring out Aurthor, Publisher, and year this was made?

Okay so i'm doing an Annotated bibliography on Teddy Roosevelt, and one of my sources is the "American National Biography (V. 18 Putman-Roush) But I need the Author, Publisher(s), and year it was published.
